Below you will find links to the paper versions of the forms that will be used for the 2018 Point-in-Time survey. Providers who regularly use HMIS will enter information there as usual. These forms are for providers who do not participate in HMIS, and for Outreach efforts to contact those who are unsheltered on the night of the count. There is also a form specific to Youth encountered during the count. Forms specifically for DV Providers will be sent directly to those agencies soon.
