Home Programs Resources Training   New Program Eligibility Form for APR in Sage Beginning June 2, 2024, Annual Performance Report (APR) submissions and resubmissions in the Sage HMIS Reporting Repository (Sage) for the Continuum of Care (CoC) Program, Youth Homelessness Demonstration Program (YHDP), and projects funded through the Special Notice of Funding Opportunity (Special NOFO) Unsheltered Homelessness and Rural Set-Asides will be required to include a new Program Eligibility form. CoC Program, YHDP, and Special NOFO funded projects may only provide housing and services to persons who are experiencing homelessness as defined by HUD. APRs submitted in Sage are reviewed by HUD, specifically “Q15 – Living Situation,” to determine if participants entering the project are experiencing homelessness based on their living situation at project entry. Many times, HUD is unable to determine if participants entering the project from non-homeless situations are eligible, and the APR is returned to the recipient to add additional information. There are situations where a simple explanation can clarify why participants may be eligible when entering from a non-homeless living situation. These situations may include when a person is fleeing a domestic violence situation and is eligible under Category 4 of the homeless definition or when a person is transferring from one permanent supportive housing program to another permanent supportive housing program that better suits their needs. The new Program Eligibility form will enable recipients to provide an explanation prior to submission of how eligibility was determined, minimizing the need for HUD to return the APR to the recipient to add additional information. This new Program Eligibility form is required for all APR submissions and resubmissions, except for the following grant component types: SSO-Coordinated Entry grants Planning grants Homeless Management Information System (HMIS) dedicated grants Additional Resources Visit the Sage page on the HUD Exchange for detailed reporting instructions for CoC Program, YHDP, and Special NOFO projects.
